Our Products
Each of our products or individual programs that comprise the Co-Health Plan have their own Nationwide Network of Professional Benefit Providers. They have contractually agreed to provide their products and services at a special discount to our Co-Health members Providers. To locate a Provider close to you call the appropriate number listed on the back of your ID Card or visit our website www.cohealthusa.com, where they are listed nationally by zip code. Of course, you can always call us at Co-Health (800-373-0881). After enrolling, the new member receives their membership package containing a Welcome Letter, a brochure explaining the benefits and how to use them, plus, their Co-Health personal membership ID Card. Members may select a Provider from the appropriate Network, make an appointment and avail themselves of the benefit services at discounted prices. All discounts are provided at point of sale or service. Co-Health does not in any way infer any preference or recommend any specific Provider over another within the Networks. A brief description of individual programs is as follows: Vision A member can go to any one of over 13,000 locations, including all of the national and regional chains plus thousands of independents. National Benefit Providers include Pearle Vision, Sterling Optical, LensCrafters, Cohen Fashion Optical, Sears and J.C. Penney. Discounts range from 20% to 60% on prescription eyeglasses and contact lenses. There is a low price guarantee plus a 100% guarantee of satisfaction (if, within 30 days of purchase, you are not 100% pleased with your new prescription eyeglasses, you may return them for a 100% refund, or new pair of eyeglasses). All items in any store are available for discount. You can also get great savings on designer sunglasses and disposable contact lenses. On average, our members save $100 to $130 on each prescription eyeglasses purchased - most people buy two pair of prescription glasses at a time, doubling their savings to approximately $250. There is never a limit on the number of purchases that a Co-Health member can make - and the Co-Health member's discount always remains the same. In addition, our Vision Program has a special Ophthalmic Benefit. Through a national network of Ophthalmologists, we provide medical care for the eyes at a 20% discount, including Lasik Surgery, which is still not covered by most Insurance plans. The Co-Health Vision Plan is available as part of the Comprehensive Co-Health Plan, as an individual selection option plan, as part of the Co-Health Group Plan for Colleges and as a Group Co-Health "stand-alone" product. The Co-Health Benefits Plan provides access to the Aetna Dental Access® Network. This Dental Network provides members with discounts of 15% - 50%* on dental procedures at over 76,000** available dental practice locations nationwide. Members simply present their card bearing the Aetna Dental Access® logo, along with payment in full of the discounted fee, for immediate savings at the time services are rendered.
Plan 500 is a Reduced Fee per Procedure Discount Plan. Administered through Healthplex® and is offered to those in the NYC, Long Island , and surrounding areas. This plan allows you to see the "usual and customary dental fees" in your area and your discounted cost. You can clearly identify your savings. The discount averages 40% - 60%. There are more than 25,000 participating dentists.

If you do not have Chiropractic coverage, this part of the Co-Health Plan can be a great benefit. If your present coverage is limited, this benefit could be very important. It is included in your Comprehensive Co-Health Plan at no additional cost. Hearing This is the finest Discount Hearing Program in the United States. You go to certified audiologists to insure you get the proper testing, fitting, etc. You select from the best hearing aids and get a free annual cleaning. Our Co-Health member's discount is 20% (10% on digital hearing aids). The Hearing benefit is included in your Comprehensive Co-Health Plan at no additional cost. Special Diabetic Program This, too, is handled by our Mail Order Pharmacy. If you qualify, there are great savings on diabetic supplies. You do not qualify if you are a member of an HMO or are covered by Medicaid. The Special Diabetic Program is included in your Comprehensive Co-Health Plan at no additional cost. 24 Hour Nurse Hotline "Peace of mind", 24 hours a day, 365 days per year you can pick up the Phone, dial an 800 number and speak to an Registered Nurse (RN) on any medical question. This is a great resource for high temperatures during the night or on a holiday, when you physician is not available. The 24 Hour Nurse Hotline is included in your Comprehensive Co-Health Plan at no additional cost. Remember - All Co-Health Benefits apply equally to you, your spouse and all your children. One membership includes your entire family living in your household - and all your children up to age 23 that are attending an accredited college.
